Sometimes all tests look “normal”: semen analysis is acceptable, hormone levels are within range, and no clear medical cause is found. Yet pregnancy still does not occur. This is often called unexplained subfertility, and it can feel just as heavy (or even heavier) than having a clear diagnosis. At Mesology Amsterdam I work specifically with this group of Subfertility Men by looking beyond standard parameters, with attention to the whole system.
What does ‘unexplained’ subfertility mean?
We usually speak of unexplained subfertility when:
- a couple has been trying to conceive for a year or longer,
- basic investigations in both partners show no obvious abnormalities,
- cycle timing and intercourse are appropriate,
- and no concrete medical cause can be identified.
This does not mean nothing is going on. It means that with the current standard tests, we cannot (yet) pinpoint the underlying reasons. Often there are subtle imbalances at several levels that together make the difference between “it should work” and “it still doesn’t”.
The limitations of looking only at semen numbers
A semen analysis measures:
- sperm count
- motility (movement)
- morphology (shape)
But it tells us little or nothing about:
- DNA damage and epigenetic quality of sperm
- subtle hormonal dysregulation
- low‑grade inflammation or immune activity
- gut and semen microbiome
- stress resilience and nervous system balance
- nutritional status, toxic burden and micronutrient deficiencies
So you can have “normal” semen parameters on paper and still not be in an optimal state to conceive and sustain a healthy pregnancy.
What does mesology look at instead?
Mesology starts from the idea that fertility is an expression of the overall state of the body: digestion, hormones, nervous system, immune function, psyche and environment. Not just the reproductive organs.
In unexplained subfertility I look at, among other things:
- Digestion and metabolism
How well do you digest and absorb food? What is your bowel pattern like? Do you have bloating, reflux, energy dips after meals? This links directly to hormone production, detoxification and supplying building blocks for sperm. - Liver function and detoxification
The liver is central in hormone metabolism, detoxifying environmental substances and processing fats. If it is overloaded, hormonal balance and inflammation can be affected. - Gut health and microbiome
Disturbed gut flora can influence immunity, hormones and inflammation, which in turn affects fertility. Local microbiota in the reproductive tract are also relevant for sperm quality. - Hormonal regulation as a system
We look beyond just testosterone: thyroid, adrenal function and central regulation in the brain all play a role. Subtle imbalances may not show up clearly in standard ranges, but can be visible through symptoms and mesological findings. - Stress, nervous system and recovery
Chronic stress and overload affect both the HPA and HPG axes, impacting hormones, sleep, recovery and fertility. Mental and emotional strain around the fertility journey is part of this. - Traditional systems (TCM/Ayurveda)
Concepts like kidney energy, ojas, agni, yin/yang and stagnation help us understand how your constitution and energy relate to reproductive strength.
Combining these layers gives a much richer, more personal picture than semen numbers alone.
WHAT DOES A TRAJECTORY FOR UNEXPLAINED SUBFERTILITY LOOK LIKE?
1. Intake: your story comes first (90min): In a first 1.5‑hour consultation we explore:
- your and your partner’s fertility story and any medical work‑up so far
- physical complaints (also unrelated to fertility)
- digestion, energy, sleep, mood, libido and stress
- diet, lifestyle, work, movement and previous treatments

2. Mesological Assessment: Using physical measurements (including testing at acupuncture points) and clinical assessment, I map out which organs, systems and regulatory processes are under strain or out of balance. I combine conventional knowledge with TCM and Ayurvedic insights to create your individual profile. Your not “the man with normal numbers results”, but a whole person.
3. Personalised Treatment Plan: Based on this, you receive a tailored plan that may include:
- targeted nutritional advice (anti‑inflammatory, microbiome‑supportive, hormone‑balancing)
- lifestyle changes (movement, sleep, daily rhythm, workload)
- supplement and herbal support where appropriate
- support for liver, digestion and detoxification pathways

Whenever possible, this plan is coordinated with your GP or fertility clinic, so that all care works in the same direction.
4. Follow-up and adjustment Plan: In follow‑up consultations we evaluate how you are feeling, how your energy and symptoms are changing and if available whether semen or blood results are shifting. We adjust the plan as needed, and align it with the phase of any ongoing IUI/IVF/ICSI trajectory.
